The authors report necropsy findings following free tenotomy of the medial rectus (MR) muscle in six postnatal monkeys. Bilateral MR tenotomy was performed producing alternating exotropia of 30-70 degrees with no adduction beyond primary position. Over follow-up, all showed reduction in strabismus angle with stabilised exotropia of 7-60 degrees. The angle reduced alongside improving adduction of 10-20 degrees typically. Necropsy was performed at a mean age of 6.5 years (3-12). This revealed that all 12 MR muscles had reattached to the globe and were firmly attached at the original insertion site by an abnormally long tendon.
